Job Description

Applications are sought from clinical pharmacists with a passion for ambulatory care and medical specialties (e.g. cardiology) for this exciting new position.

This role involves leading the Ambulatory Care and Medical Specialties pharmacy team and delivering cardiology pharmacy outpatient services to the Ballarat Health Services’ cardiology department. You will optimise medication management for patients attending the cardiology pharmacy outpatient service by undertaking consultations with patients prior to the patient’s cardiologist outpatient appointments (primarily by telehealth). This consultation includes capturing a best possible medication history, undertaking adherence assessment, reviewing other markers of cardiac health and assisting the patient to improve their cardiac health literacy. In addition, you will attend and support the multi-disciplinary heart failure clinic.

Additionally, as a Team Leader, you will be expected to provide leadership, supervision and support to the Medical Specialties Clinical Pharmacy Team, and participate in activities relating to the management and expansion of the Ambulatory Care and Medical Specialties pharmacy services.
