Position 1: Partnered Pharmacist Medication Charting (PPMC) Project Officer and Clinical Pharmacist (Grade 2)
Location: Grampians Health Ballarat

Position 2: Patient Flow Pharmacist (PFP) Project Officer and Clinical Pharmacist (Grade 2)
Location: Grampians Health Ballarat
Employment type: Permanent, Part time
Hours per fortnight: 48 hours per fortnight

About the roles

We are seeking applications from energetic and enthusiastic pharmacists with hospital and clinical experience for these permanent full time and part time project officer/clinical pharmacist roles. These roles provide exciting opportunities for pharmacists seeking influence and shape expanded scope of practice roles for pharmacists in the Grampians Region.

The PPMC project officer and clinical pharmacist role is primarily responsible for embedding a sustainable PPMC service across the Internal Medicine Service (IMS) teams at Grampians Health Ballarat. The first 12-18 months of the role will focus on further establishment and embedding of the existing PPMC workflows within the General Medicine Units. After this time the project officer will be responsible for expanding PPMC workflows across all IMS units (currently includes cardiology, neurology, oncology and gastroenterology). Once PPMC services are embedded throughout the IMS teams, the project officer role will predominately convert to a clinical pharmacist role working within IMS with some ongoing PPMC program oversight duties.

The PFP project officer and clinical pharmacist role is primarily responsible for embedding a sustainable PFP workforce model for the Internal Medicine Service (IMS) at Grampians Health Ballarat. The PFP role aims to expedite patient flow throughout the organisation where medication related processes are involved, particularly at transitions of care. The PFP will work collaboratively with Patient Flow Coordinators, IMS team clinical pharmacists and medical officers. The first 12-18 months of the role will focus on establishment and embedding of PFP workflows within the IMS specialty units (currently includes cardiology, neurology, oncology and gastroenterology). After this time the project officer role will be responsible for expanding PFP workflows to the General Medicine Units. Once PFP services are embedded throughout the IMS teams, the project officer role will predominately convert to a clinical pharmacist role working within IMS with some ongoing PFP program oversight duties.

Each project officer role will work closely with the Clinical Pharmacy Team Leaders for General Medicine and Medical Specialties and the Deputy Director of Pharmacy - Clinical Services.

Professional development is supported through regular in-service education including journal club meetings and support for seminar/conference attendance is available. In addition, professional support is provided through team-based structures as well as mentoring programs.

Renumeration will be award-based according to experience. Salary packaging and allowances for higher qualifications may provide additional salary benefits.

Skills and experience

  • Currently hold general pharmacist registration with AHPRA
  • Acute inpatient clinical pharmacy experience
  • Understanding of barriers and enablers to patient flow in acute healthcare
  • Ability to effectively engage stakeholders to achieve desired outcomes
  • Ability to effectively plan, organise, prioritise and manage time with ability to deliver outcomes to agreed timelines
