Ballarat Health Services, proudly part of Grampians Health invites applications for a Pharmacist with hospital experience for this permanent, part time position at Ballarat Health Services Pharmacy Department.

Applicants must have, or be eligible for, registration with the Pharmacy Board of Australia.

This role will primarily involve the provision of specialist clinical pharmacy services to the Dialysis Unit at Ballarat Health Services, including contributing to the care of dialysis patients as a member of the multidisciplinary team, serving as a speciality reference point for the broader clinical pharmacy team and implementing service improvements and innovations to advance clinical pharmacy care for dialysis patients throughout the Grampians Region. Remuneration will be award-based according to experience.

Suitable applicants will be provided opportunities to undertake credentialing and practice Partnered Pharmacist Medication Charting (PPMC).

Professional development is supported through regular in-service education including journal club meetings and support for seminar/conference attendance is available. In addition, professional support is provided through team-based structures as well as formal education programs including the SHPA Residency Program.
