About the facility  

The Pharmacist will provide pharmacy services to ensure safe, appropriate and efficient drug use within Barwon Health and on transfer to a community setting. The facility provides a pharmacy service including resolving medication related issues, supplying medications, completing the Medication Reconciliation Process, providing medicines information, counselling patients and assisting the discharge process.

The Pharmacy Department has a strong workplace culture of high achievement, positive teamwork and enjoying work. You will work with an innovative multidisciplinary team who maintain extensive knowledge and experience.

About the role

The role will ensure appropriate drug utilisation through implementation of hospital policy and formulary guidelines/restrictions, and through liaison with medical and nursing staff.
