Job Description

There is an exciting opportunity for an experienced Pharmacist (Grade 3) to join South West Healthcare as a Project Pharmacist as part of the Better Care Victoria (BCV) Project.

This 12 month full time position will work directly with both the Pharmacy Director of South West Healthcare, and BCV project Pharmacy Executive, in order to facilitate completion of the BCV Pharmacy Project outcomes which include:

  • Reviewing and implementing medication policies in the region
  • Review of CQI data from ‘Smartpumps’ used at South West Healthcare, Portland District Health, Colac Area Health and Western District Health Service
  • Review and implement appropriate dataset for Moyne Health Service
  • Provide support in the implementation of ‘Ward box’ deliveries as needed
  • Provide support as needed to the project team to complete BCV outcomes

Hours of Duty:

08.30am -17.30pm, Monday to Friday (38 Hour week) (No weekend hours required.)

Primary Objective;

  • Working directly with both the Pharmacy Director of South West Healthcare, and BCV project Pharmacy Executive, in order to facilitate completion of the BCV Pharmacy Project outcomes

Specific Responsibilities, Duties and Skills;

  • Reviewing and implementing medication policies in the region

A.Identify gaps in policies, and make recommendations to reconcile same.
B.Liaise with all stakeholders in the development and implementation of regional policies and procedures.

  • Review of CQI data from ‘Smartpumps’ used at South West Healthcare, Portland District Health, Colac Area Health and Western District Health Service.

A.Benchmark CQI data from all four sites, as directed by Project Executive.
B.Identify areas for consolidation at all four sites, with the goal of implementing one regional dataset.

  • Review and implement appropriate dataset for Moyne Health Service

A.Includes education and support to nursing staff as required.
B.CQI report to be generated at 2 months and 6 months after dataset implementation, with appropriate feedback provided to Moyne Health Service.

  • Provide support in the implementation of ‘Ward box’ deliveries as needed
  • Provide support as needed to the project team to complete BCV outcomes.

  • Holds a recognised degree in Pharmacy from an approved tertiary institution.
  • Preferably has at least 6 years experience in a broad range of Pharmacy services in the general hospital field.
  • Is able to work in and promote a multidisciplinary team environment.
  • Has well-developed interpersonal skills.
  • Has a current Victorian Driver’s licence
