Job Description

Seeking an experienced Grade 2 Clinical Pharmacist to work part time in Pharmacy across Western Health sites with outstanding clinical and interpersonal skills that demonstrate a willingness to develop and maintain their knowledge

Duties:

  • To ensure patient outcomes are optimized by working to achieve the best Quality Use of Medicines
  • Support prescribers and patients to maximize clinical benefits of drugs and minimize unwanted reactions
  • Minimise treatment costs by optimizing drug prescribing and supply
  • Provide expert clinical pharmacy advice to range of specialties
  • Actively participate and support educational and operational activities of the department

Selection Criteria:

  • Bachelor of Pharmacy or equivalent
  • Eligible for general registration with AHPRA
  • Experience in providing clinical pharmacy services to a broad range of specialty areas
  • Well developed written, oral and interpersonal communication skills, and the ability to work co-operatively with hospital staff and patients
  • Sound knowledge and understanding of all relevant aspects of pharmacy services, particularly a demonstrated ability to practice in accordance with the SHPA Guidelines
  • Experience in contributing to practice improvement projects
  • Experience in supervising and providing education and training to pharmacy staff and students
  • Works with a minimum of supervision
  • Ability to work as part of a team and contributes to projects and initiatives
  • Strives for maximum attendance/punctuality
  • Fosters and maintains working relationships
  • Demonstrated ability to adapt to a changing environment and demonstrate initiative within it
  • Ability to understand and adhere to Western Health Policies and Procedures
  • Active contributor to committee meetings
  • Practices collaboratively as part of a multidisciplinary health care team
  • Demonstrates an ability to identify and solve problems
  • Demonstrated ability to meet time constraints and to determine workload priorities
  • Possesses well-developed computer skills
